Documents & Forms

Please see the "New Members" page for a list of all the required forms for the 2022/2023 season.  Forms used in previous years are listed below and may or may not be used for the current year.

  • Community School Form

    • Please bring this form to camp with you on the first day. If you do not have access to a printer, we have copies at the school & they can be filled out that morning. This form does not need to be notarized.​

  • Volunteer Level 2 Clearance Form

    • Any parent wanting to be a Level 2 clearance volunteer will need to fill out the form attached. In the event that the band has an overnight trip, we will need to have chaperones cleared on this level. Completed forms should be emailed to wattsj@santarosa.k12.fl.us or turned into the front office. Once approved, Level 2 is good for 5 years.

 

  • SRC Off-Campus Field trip form

    • ​​Must be completed by all students before traveling with the band to any event.

    • These will be handed out during camp and will need to be returned before the first off-campus event.

 

  • Absence Request Form

    • This form will need to be printed and returned to the Band Director as the need arises  

 

  • Football Program Ad Request Form

    • Advertise your business in the 2022 GBHS Football Program!
       

  • Student Salute Order Form

    • Student Salutes are a great way to honor and support your child and/or graduating Senior.  We offer Student Salute Ads to the Football, Cheerleading, Marching Band and Color Guard members.
